Hospital Discharge Service
We get the right care for patients coming into and leaving hospital. Tailored care, so patients can be free to live as independently as possible.
Once out of hospital, we all want to get back to living well as soon as we can. And that’s no different for an elderly person, or someone with dementia or cerebral palsy.
As a social worker with our Post-Hospital Review Team, you’ll work with patients, families and professionals to make that happen – by reducing patients’ chances of readmission and carrying out assessments and reviews in residential and nursing accommodation, as well as people’s own homes.

Life’s nothing without a challenge – and you’ll certainly face challenges here. It’s a complex job, but it’s also extremely rewarding.
Each team comprises community care officers, social workers, a support officer, team manager and deputy manager. Some hospitals have advanced practitioners, too.
We’re able to provide a seven-day service because we work consecutive Saturdays and Sundays once every 8 weeks. We get time off in the weeks before or after to make up for this.

What we offer
Just as life requires risk to get the most from it, work requires regular training. We offer plenty of that here, as well as a career progression scheme, regular practice sessions to reflect on recent cases, 1-1 supervision and team meetings. You’ll work from home and travel to offices, hospitals and community settings as needed.
